Best Home Health & Hospice is looking to hire dependable, flexible and independent CNAs

Responsibilities:

Provides services in accordance with the plan of care.
Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
Provides those services requiring specialized nursing skills.
Prepares clinical and progress notes for each patient visit as per Agency policy.
Communicates changes in the condition and needs of the patient.
Understands and adheres to established Agency policies and procedures.
On-call rotation as assigned.

Requirements:

Associates Degree in Nursing, Active RN license, Driver’s License and Personal Vehicle.

Best Home Health & Hospice is looking to hire dependable, flexible and independent CNAs

Responsibilities:

Work under the direction of a Registered Nurse to provide various personal care and related professional services in accordance with an established plan of care.
Provide for the personal needs and comfort of patients in their homes.
Perform personal care activities that assist the patient with daily living which include but are not limited to personal hygiene (bathing, care of mouth, skin and hair) and meal preparation
Perform homemaking activities to provide a safe clean living environment which include (but are not limited to): general housekeeping and cleaning, changing bed linens, doing laundry, grocery shopping, taking client to doctors appointments.
Provide ambulation-assist as needed.

Requirements:

Active CNA license, Driver’s License and Personal Vehicle.
